Table 9.1 Amenorrhea

Etiology

Amenorrhea is described as the absence of menstruation. It is considered failure of hypothalamic-pituitary-gonadal-uterine interaction to produce cyclic changes in the endometrium that typically result in menses. Young girls who have not started menstruating prior to age 15 or 16 are considered to have delayed menarche or primary amenorrhea, while women who experience an absence of three or more consecutive menses are considered to have amenorrhea. Causes include pregnancy, menopause, medications, low body weight, excessive exercise, stress, anatomical abnormalities, hypothalamic, pituitary or other endocrine dysfunction, ovarian failure, or genetic defects. Complications of not having regular menses include infertility and osteoporosis.
